R-24-09VILLAGE OF DEERFIELD RESOLUTION NO. 2024- R-24-09 A RESOLUTION AUTHORIZING A GRANT TO FAITH IN ACTION WHEREAS, Article VII, Section 10 of the 1970 Illinois Constitution authorizes the Village of Deerfield ("Village') to contract with individuals, associations, and corporations in any manner not prohibited by law or ordinance; and WHEREAS, Faith in Action is an Illinois not -for -profit that provides transportation and other social services (collectively, "Services') to senior citizens and other residents living in the Village and nearby communities; and WHEREAS, Faith In Action's Services are essential for numerous Village residents; and WHEREAS, each year the Village provides a grant to Faith In Action to assist Faith In Action to provide Services to Village residents; and WHEREAS, the Village Board has determined that it is in the best interests of the Village and its residents to provide Faith In Action a grant of $12,000.00; NOW, THEREFORE, BE IT RESOLVED BY THE VILLAGE BOARD OF DEERFIELD, LAKE AND COOK COUNTIES, ILLINOIS, as follows: SECTION 1: RECITALS. The Village Board hereby adopts the foregoing recitals as its findings, as if fully set forth herein. SECTION 2: APPROVAL OF GRANT. The Village Board hereby approves a grant to Faith In Action in the amount of $12,000.00. SECTION 3: AUTHORIZATION TO EXECUTE GRANT. The Village Board authorizes and directs the Village President, the Village Manager, the Village Clerk, and the Village Finance Director to execute and attest, respectively, on behalf of the Village, all documents necessary to issue the grant approved in Section 2 of this Resolution. Suite 205, Highland Park, IL 60035 Village of Deerfield Funding Request 2024 August 23, 2023 To: Kent Street From: Robbie Boudreau Subject: 2024 Funding Request The Agency mission is to promote the independence and quality of life of older and disabled adults by improving their access to healthcare and social interaction. Funding Request: $12,000 Funding Rationale: Total Number Agency Clients Served FY 2022-2023 472 Number of Agency's clients who live in Village of Deerfield: 111 (24%) Total Agency Budget for FY 2023-2024 $125,000 Funding Request for Village of Deerfield for FY 2023-2024 $12,000 (100/6) Funding Request is based on Faith in Action's "Fair Share" Funding Strategy: Village of Deerfield is the only community that is not meeting their "fair share" quota as they are only receiving $10500 in community specific funding. Most of this funding is coming from West Deerfield Township ($8750.) All other communities perennially meet their "fair share" and even exceed "fair share" quotas which means they are essentially subsidizing our cost to serve Deerfield residents. The reason we are coming forward now is that the grants we receive are increasingly asking us to tie our funding to more exact and narrower geographic areas. For your benefit, we have reviewed our historic case logs and can share the following Deerfield -only statistics with you. Since 2005, FIA volunteers have provided the following to Deerfield's disabled and older adults: 6000 free rides 12000 hours of free, volunteer caregiving 2400 free service errands 36000 free friendly visits and phone calls 7200 hours of free social programming Southeast Lake County Faith in Action Volunteers Annual Service Report July 2022 June 30, 2023 Mission The mission of Faith in Action is to improve healthcare outcomes and prevent social isolation of older adults 65 years and older. Programs and services fill Ws in transportation, caregiving, behavioral health and recreational services at the local, county and state level. Background Faith in Action Volunteers has been serving disabled and older adults living in Deerfield since 2004. We also serve seniors living in Buffalo Grove, Riverwoods, Bannockburn, Lincolnshire, Highland Park, Highwood, Lake Bluff & Lake Forest. There are approximately 26,000 older adults 65+ in our service area and 3200 in the Village of Deerfield. (Source: US Census 2020) Our target population lives on a fixed income and is aware that they need to seek help from others if they are to continue to remain independent in their home. 80% of the adults we serve have income under $49,999. In addition, 90% of the adults we serve have challenges in one or more of the six primary functional areas: mobility, hearing, vision, communication, cognition, and/or self -care. The reason for this rate of usage is because there is a bigger gap in senior services in Deerfield than other communities. Please consider joining us at a Social Services Group that meets on the 4`h Tuesday of the month at West Deerfield Township at 12:00pm. The group would be happy to share thoughts about the strengths and challenges concerning senior services in Deerfield. Members include all social service professionals who work with vulnerable populations in the Village. Gan -filling Proerams and Services TRANSPORTATION SERVICES provide safe, reliable medical transportation for frail, disabled and cognitively impaired adults who require a higher level of guidance and supervision when leaving home. Companion drivers remain with the older adult throughout the time they are away from home, including during the appointment if desired. Our GoGo Service (a partnership with Uber and Lyft) provides safe, reliable transportation beyond the scheduling and geographic limitations of what existing community resources offer including early morning, weekends, longer distances, and the number of rides allowed per week. CAREGIVING SERVICES fill gaps to address waiting lists and increasingly stricter income guidelines for state and federal programs such as Catholic Charities and the Illinois Dept of Aging. Volunteer Services provide support with some of the activities of daily life such as grocery shopping, medication management and meal preparation. BEHAVIORAL HEALTH SERVICES fill gaps in services to reduce loneliness and improve emotional well- being. In -home Counseling addresses barriers the Very Old have accessing mental health resources. SOWN (Supportive Older Women's Network) brings together older adults who live alone to create a family of friends for mutual -support and friendship. Phone Friends is a telephone companionship program for socially isolated seniors that are not comfortable with in -person contact. ACES (Adult Caregiver Education & Support) is a bi-weekly support group that offers family caregivers an opportunity to find mutual support and get breaks from the demands of family caregiving. RECREATIONAL SERVICES fill the gaps between what local senior centers offer and the needs of physically, mentally and psychosocially compromised seniors in the community. Faith in Action programs offer volunteer companionship, transportation and program modifications so mobility -challenged, hearing and visually impaired, socially anxious and non -drivers can stay more social connected to their peers. Free, professional caregiving services are available without an income eligibility or an exhaustive intake process. Community Care Program: Catholic Charities Lake County provides state -funded in - home and adult day services for financially eligible older adults. SUPPORT GROUPS A.C.E.S: The Patty Turner Center hosts a caregiver support group for family members caring for a spouse, parent, or other older relative. Meetings are the 2nd and 4th Wednesday at 1 pm. LENDING CLOSET The Patty Turner Center has a variety of durable medical equipment available free to residents of Deerfield, Riverwoods, Bannockburn, and Lincolnshire. PERSONAL & HOME SAFETY Special Concerns Person: The Premise Alert Program is a program that notifies the police that you have someone with special needs living in your home. Fill out a simple form and the information will be logged into the 911 system and a copy will be kept at the police station in case of an emergency. ADVISORY SERVICES Health Insurance.: The Senior Health Insurance Program (SHIP) provides free, individual, counseling services by trained volunteers at most local senior centers and is dedicated to educating people with Medicare about insurance options that will save you money and maximize coverage.
