RESOLUTION NO. 2022- R-22-11
A RESOLUTION AUTHORIZING AN AGREEMENT WITH FAMILY
SERVICE OF LAKE COUNTY TO PROVIDE PROFESSIONAL
COUNSELING FOR ALL EMPLOYEES OF THE VILLAGE
WHEREAS, the Village of Deerfield ("Village') is a home rule municipality in
accordance with Article VII, Section 6 of the Constitution of the State of Illinois of 1970; and
WHEREAS, Family Service of Lake County, an Illinois not -for -profit corporation
("Family Service', provides counseling, education, and support services to individuals and
families ("Counseling Services'); and
WHEREAS, the Village desires to offer Counseling Services to Village employees,
their spouses, and their dependents under the age of 26; and
WHEREAS, the Village desires to enter into an agreement with Family Service to
provide the Counseling Services ("Agreement's; and
WHEREAS, the Village Board has determined that it will serve and be in the best
interests of the Village and its residents to approve the Agreement with Family Service
pursuant to this Resolution;
NOW, THEREFORE, BE IT RESOLVED BY THE VILLAGE OF DEERFIELD,
LAKE AND COOK COUNTIES, ILLINOIS, as follows:
SECTION ONE: RECITALS. The foregoing recitals are incorporated into, and
made a part of, this Resolution as findings of the Village Board.
SECTION TWO: APPROVAL OF AGREEMENT. The Agreement by and
between the Village and Family Service is hereby approved in substantially the form
attached as Exhibit A.
SECTION THREE: AUTHORIZATION AND EXECUTION OF THE
AGREEMENT. The Village President and the Village Clerk are authorized and directed to
execute and seal, on behalf of the Village, the Agreement with Family Service.
SECTION FOUR: EFFECTIVE DATE. This Resolution will be in full force and
effect from and after its passage, approval, and publication in the manner provided by law

AGREEMENT
THIS AGREEMENT entered into this 1st day of January 2022, and extending to
midnight, December 31, 2022, by and between THE VILLAGE OF DEERFIELD, a
municipal corporation, and the FAMILY SERVICE OF LAKE COUNTY, a not -for -profit -
corporation.
WITNESSETH:
The parties hereto, upon the terms and conditions hereinafter set forth, mutually agree
as follows:
1. FAMILY SERVICE OF LAKE COUNTY agrees to provide professional
counseling for all employees of THE VILLAGE OF DEERFIELD, their spouses
and dependents up to the age of 26.
2. FAMILY SERVICE OF LAKE COUNTY agrees to provide to the Village
monthly accounting of time used a monthly basis. The accounting shall deal only
with units of time used without naming such individuals.
3. FAMILY SERVICE OF LAKE COUNTY agrees that any of the Village's
employees, spouses and dependents up to the age of 26 will receive counseling
within a 48 hour period of a request thereof during the work week.
4. FAMILY SERVICE OF LAKE COUNTY agrees to arrange, at no extra charge,
a mid -year conference between themselves and a Village staff member
appointed by the Village manager to determine if there are any major categories
or recurring themes of which the Village should take recognition in terms of its
operations.
5. THE VILLAGE OF DEERFIELD agrees to pay FAMILY SERVICE OF LAKE
COUNTY for the above listed services rendered, at an hourly rate of $130 per
hour, with such payment to be made monthly upon the receipt of a bill from
FAMILY SERVICE and in accordance with the following guidelines, provided that
the obligation shall not exceed $11,000 during the term of this agreement.
These services will also include:
5.1 Individual, marital, family therapy
5.1.1 Full payment for the first 12 therapy sessions ($130)
5.1.2 75% of the 13th to 24th therapy sessions and ($98)
5.1.3 50% for all subsequent sessions. ($65)

6. The employee will pay based on ability to pay up to a maximum of the portion
the Village does not pay. (i.e., 25% of the second twelve sessions for individual
therapy and 50% of the individual sessions after the 241h)
7. Any In -Service Training programs to be provided by the FAMILY SERVICE to
Village personnel will be arranged through the year and will be billed at the rate
of $100 per hour.
8. FAMILY SERVICE will provide consultation to a Management staff at $130 per
hour to assist in supervisory and/or referral help.
9. If needed, the FAMILY SERVICE may refer an employee for one of the
following services to a subcontractor:
9.1. Budget Counseling and Debt Management:
Payment for up to two sessions will be assumed by the City for these additional
services.
9.2. Alcohol and or substance abuse evaluation.
Payment will follow the same terms as indicated in item number 5.
10. This agreement will be in full force and effect for a period of twelve months,
but may be canceled on 60 days written notice by either party.
